I’ve been bumping this thread (Thread 126584) since 2011 (on my old account that I’m now locked out of), trying to get someone to cough this one up. I wouldn’t get your hopes up.

That being said, if someone miraculously does have this and is willing to upload, please, by all means, be my guest.

I was hoping it was an oversight, but it seems like it’s simply never been done. Shame.
