Well one of the scary ones for me was "Wraith Intrusion" (I think that is the name of the track) from StarGate Atlantis soundtrack. There is one moment where the sounds goes up in millisecond. I jumped from my seat when I hear that. On the same disc is also another "Wraith Lair" (I think) that is really spooky too.
I have some more evil/scary music (SW soundtracks for example), but I don't remember them all. So I might be back when I will listen to one of them ;-)

As for most haunting... hm. No idea. I really don't get spooked when playing a game. Now if it were the Texas Chainsaw movies, the first remake scared the shit outta me when I watched it in the theaters. Maybe because it was so loud.

Someone already mentioned Neon Genesis Evangelion, but I'm going more specific: "Do You Love Me?" from Evangelion album III, track 14. It was creepy in the show, and is creepy as a standalone track.

Can't believe nobody suggested Vagrant Story. It's like 55 tracks and only about five have anything really bright in them. I'm not the most knowledgeable person, but it's probably the single most unrelentingly dark game soundtrack outside of the horror genre.

To me Resident Evil probably has the creepiest music ever. The mansion kitchen level in the original is supremely freaky. Some of the later music is dark without being creepy (still great, though, #4 has some of my favorite VG music of all time). Silent Hill has some disturbing stuff (I generally prefer Broken Notes to the original music, though), and Dead Space has some great tracks.
