You need 100 Curse Spikes and a level 100 Siren to make one. They're used to teach Quistis the ShockWave Pulsar Blue Magic, and can also be refined into a Luck J-Scroll or 100 Ultimas.

No, that's Ultima Weapon, Omega Weapon is in Ultemecia's Castle. You fight Omega Weapon as follows:
